Families with students enrolled in the Carlisle Schools for the 2012-2013 school year will need to complete the following forms:

• Student Information and Registration Form
  • Instructional Materials fee amounts are included on this form.
• Health Form - Preschool/ Kindergarten
• Health Form - Elementary (Grades 1-5)
• Health Form - Middle School/ High School (Grades 6-12)
• Language Survey Form (New Students Only)
• Dental Health Screening Form (Kdgn. & 9th Grade Students Only)
• Denial of Release of Student Information Form (New form available soon)
• Student Record Request Form (New Students Only)
• Transportation Forms
  • Students who only ride the shuttle bus to Hartford Upper Elementary or the Middle School will not need to complete a transportation form.
  • Transportation Form Grades K-12
  • Transportation Form Kindergarten-Prep Only
  • Transportation Form Preschool Only
• Athletic Physical and Insurance Waiver Form
  • This form is required for all students in grades 7-12 participating in athletic activities.
• Consent for Medical Treatment
